	This form should be completed for reporting a student absent in Years 7, 8 and 9 or to advise us of a future absence. If you are reporting for more than one child please enter the details for the first child and submit the form before entering the details for the subsequent child.
	When the information is submitted it is for the school to classify the absence as either authorised or unauthorised and we may request further information from you to enable us to do this. We will do this in line with Government guidance, BCP guidance and our school policy.
	When you submit this form, it will not automatically collect your details like name and email address unless you provide it yourself.
